We are seeking a dedicated and empathetic PRN Licensed Practical Nurse (LPN) to join our Hospice team. The ideal candidate will be passionate about delivering personalized and supportive care to patients and their families in the comfort of their homes.

What You'll Do: Provide skilled nursing care to hospice patients in their homes or a facility. Assess and monitor patients' physical and emotional needs. Collaborate with the interdisciplinary team to develop and implement individualized care plans. Educate patients and families about hospice care and provide emotional support. Maintain accurate and timely patient records. Ensure the comfort and dignity of patients during their end-of-life journey. About You An active LPN license in the state in which practicing. Previous experience in hospice care is preferred but not required. Excellent communication and interpersonal skills. Compassion and empathy for patients and their families. Ability to work collaboratively in a multidisciplinary team. Strong organizational and time-management skills.
